What are your health and wellness needs?
You’ll be asked a few questions on a short Initial Questionnaire to help us tailor our suggestions. We will schedule and complete a quick 15 minute No Charge Discovery Call to determine if we are a good fit for each other.

Personalized suggestions.
If you decide to proceed with care at Align Functional Med, you will complete an extensive Initial Intake Form and we will schedule a 60 minute intake visit where you will receive advanced functional medicine lab and supplementation recommendations with resources if applicable.

Continuation of care.
Upon your completion of the Initial Intake Visit and advanced functional medicine labs, we will schedule a 90 minute Report of Findings visit and biweekly follow up visits for a total of 3 months included in our signature Align Me program so we can work closely together to optimize your health and wellness outcomes and ensure longstanding relief of symptoms and overall vitality.
